Free and Reduced Lunch Info

Families can now apply online for free/reduced lunch benefits by logging into their Infinite Campus portal account. Click here for instructions. Paper copies of the application form are available in both school offices, as well.

 

Families need only submit one application per household, even if students attend both Maple Dale and Indian Hill Schools. The application must be filled out completely in order to certify your child(ren) for free or reduced-price school lunch. Application for free/reduced lunch benefits can be made at any time throughout the school year.  

 

Paper applications should be returned to:  MDIH School District, 8377 N. Port Washington Road, Fox Point, 53217  Attn: Sue Simpson Bark. Applications are processed daily and applicants are notified in writing whether or not they qualify for free/reduced-price meals.


Households that qualify for free/reduced price meals can choose to used that designation to waive or reduce school registration and field trip fees, as well as PTO-sponsored programs. Parent/guardian may indicate this as part of online application.  Otherwise, they must sign a Consent to Share form and return it to: MDIH School District, 8377 N. Port Washington Road, Fox Point, WI 53217, Attn: Sue Simpson Bark.
