Welcome to the Maple Dale-Indian Hill School District. To register for district schools, you must be a district resident or accepted through the Open Enrollment process. To check residency or view district boundaries, please

1.  Complete online enrollment. Before beginning the online process, please make sure that you have the following information available:
 

  • ​​Household information (address and phone number)

  • Proof of residency (two forms from linked list)

  • Student information (date and place of birth, medical and health information)

  • Parent/Guardian information (home/work/cell phone numbers, email addresses)

  • Emergency contact information (names and contact information for a minimum of two individuals you will authorize to be notified in case of emergency)

 

2.  Present the following documentation to the school office:

  • Child’s birth certificate*

  • Child’s immunization record*

* These documents are not kept by the school
