Welcome to Maple Dale-Indian Hill School District Athletics

To participate in any school sports students must have the following documents on file:

Athletic permit signed by a medical doctor and turned into the student office
Review of Concussion, Head Injury, and Sudden Cardiac Arrest Information Sheet
Parent Concussion, Head Injury, and Sudden Cardiac Arrest Agreement completed and turned into the Maple Dale office
$50 athletic fee paid before the start of the season using Infinite Campus Parent Portal
